SICKNESS CERTIFICATES

If you are off work for one to two days no certificate is required.

For three to six days, you need to complete a “self certification” form available from your employer.

If you are off work for seven days or more, you will require a doctor's certificate. For this to be issued the doctor will have to see you.

(If you require a certificate for less than seven days a private certificate will be issued for which a fee is payable).
